What is a Care Manager?
A relatively new healthcare
function, “care managers” have expertise in understanding and utilizing the
health care system to maximize benefits to the individual facing serious illness
or debilitation.
Whether
the patient is at home or in an institutional setting, care managers can evaluate
their needs, put arrangements in place, and monitor the quality of services
delivered.

Susan
Johnson, Director of Care Management Associates, is a Registered Nurse in
the District of Columbia, Maryland, and Virginia.
A Fellow of the Washington Area Geriatric Education Center Consortium, Susan serves on the Boards
of the Grass Roots Organization for the Well-being of Seniors and Joseph’s
House Hospice. She is also a Committee
Member of the D.C. Partnership to Improve End-of-Life Care. Susan has been a guest speaker for the Alzheimer’s
Association and does speaking and training on topics including care of the patient with dementia,
care for the caregiver, and how to deal with terminal illness. She has expertise in chronic long-term degenerative
diseases including Alzheimer’s Disease, Parkinson’s Disease, ALS, and HIV
Disease. And, she has years of experience
in helping those facing terminal illness.
Her clinical experience includes positions as
Patient Care Coordinator at the Washington Home and Hospice; as Clinical Manager
of the Hospice of the Visiting Nurses Association of Washington; and as Clinical
Manager of Damien House and Joseph’s House, both residential hospice programs
serving homeless and indigent people in the District of Columbia.
She graduated with honors from the Marymount University School of Nursing
in 1995 and received a Bachelor of Arts degree from the American University
in 1974.
In
addition to nursing, Susan has a background in business and government relations.
She served as a political appointee during the administration of President
Jimmy Carter and as a lobbyist at the U.S. Conference of Mayors and National
League of Cities. An active volunteer at the Jewish Community
Hospice in Montgomery County, Susan also served on the Montgomery County HIV/AIDS
Task Force, appointed by the County Executive and was a member of the Board
of Directors of the Episcopal Caring Response to AIDS.
